Download A Multidimensional Outcome Evaluation System For Chronic Low Back Pain books, Chronic low back pain (CLBP) is a complex and costly condition. Given rising health care costs, it is now essential that the efficacy of CLBP interventions be rigorously documented. Unfortunately outcome research in chronic pain has suffered from theoretical and methodological limitations. In an attempt to address these limitations, this dissertation provided an account of the conceptualization, development, validation and potential applications of a new outcome evaluation system. The multilevel system is modeled after Spilker's (1990) conceptualization of health outcomes. The bottom level provides an evaluation of unidimensional outcomes in terms of standard units of pain, disability, and depression. The next level aggregates these dimensions together into multidimensional profiles. The top level provides a summary index by averaging standard scores across each outcome dimension. The development of this system was accomplished over a series of smaller studies focused on specific conceptual, developmental, validation, and application objectives. Data from four independent treatment samples were examined. Subjects were CLBP patients and the interventions included: standard medical care (n = 95), a behavioral medicine intervention (n = 75), a functional rehabilitation program (n = 42), and a pharmacological intervention (n = 63). Results from developmental studies established standardized outcome evaluation units and indicated that a combined reference sample of CLBP patients and healthy controls could be employed to evaluate a broad range of CLBP outcomes. Results from the validation studies indicated that the system was related to quality of life, health care utilization, lumbar strength and range of motion. Change as measured by the system was related to change in measures of outcome specific to CLBP (lumbar strength and clinician's ratings of CLBP improvement), not to global health measures (quality of life and health care utilization). Five applications of the system were examined in an empirical demonstration. Specific applications included: (1) Evaluation of referral patterns and sample incompatibility. (2) Establishing base rates of change. (3) Evaluating treatment efficacy. (4) Detecting differential patient response patterns. (5) Establishing patient-treatment matches. In summary, this new system shows considerable promise and addresses a much needed approach to the evaluation of clinical outcomes and treatment efficacy for this population.
